You are in the right place to securely report a concern involving suspected misconduct, unethical behavior, policy violations, fraud, safety issues, harassment, or other matters that should be brought to the Organization's attention.

Your report plays an important role in helping the Organization identify concerns, respond appropriately, and promote a safe, ethical, and accountable environment.

This is not an emergency reporting system. If there is an immediate threat to the safety of any individual or the Organization, or if emergency assistance is needed, contact 911, local emergency services, or follow your organization's emergency response procedures immediately.

Before You Begin

• Completing a report typically takes 5–10 minutes. Once you begin your report, you will have 24 hours to complete and submit it.
• If your report is not submitted within 24 hours, your reporting session will expire. Information entered before the session expires may be retained and, in some cases, may be made available to the Organization as an incomplete report. If you decide that you do not want any information submitted, please remove the information you entered before leaving the reporting session.
• You may submit your report anonymously. If you wish to remain anonymous, do not include your name or other information that could reasonably identify you, unless you choose to do so.

What Happens Next?

After you submit your report:

• You will receive a unique Case ID and PIN. Please save this information in a secure location. You can return at any time to check the status of your report or confidentially communicate with us or the Organization.
• You may receive follow-up questions from the Organization and/or Verifi. Responding to those questions, when possible, can help the Organization better understand and address your concern.
• You can securely exchange messages throughout the life of your report while maintaining your chosen level of confidentiality. Your Privacy

Verifi securely receives your report and makes it available to the Organization for review. If you request anonymity, every reasonable effort will be made to protect your identity to the fullest extent permitted by law. However, information you include in your report or facts learned during any review or investigation could unintentionally reveal your identity.
